I have a TB gelding that i cant put weight on. hes been wormed recently and he eats well , what should i do?

he eats about 5lbs of a weight gaining eques grain and 3 flakes a day.. is this enough for him, hes about 15.3 HH and 8 years old? is there another underlining issue i might be missing?
